• Elementary School Families: PLEASE NOTE the tuition assistance process has changed since last year. Please read the following carefully.

  The Private School Aid Service Elementary School Student Aid Form 2004-2005 in English and Spanish is available on this website for downloading as well as from any Catholic elementary school in the Archdiocese.   The completed form, along with a signed copy of the parent(s) or guardian(s) 2003 (NOT 2002) federal tax return(s) and a check or money order for $17.00 payable to PSAS, must be postmarked by February 18, 2004. Any forms received by PSAS postmarked after this date will not be processed. Mail form to PSAS, PO Box 770728, Lakewood, OH 44107-0034.
